A parent or legal guardian must read and sign before registration is complete.
1. Liability waiver and release
Basketball is a physical, competitive sport. Camp activities, including drills, conditioning, combine testing, scrimmages and games, involve risks such as sprains, fractures, concussions and other injuries, some of which may be serious. I confirm my player is in good health and physically able to participate, and I voluntarily accept these risks on her behalf.
To the fullest extent allowed by law, I release, and agree not to sue, Her Prep Academy, its owners, coaches, staff, volunteers and the host facility (Power Sports Academy) for injury, illness, loss or damage arising from my player's participation, except where caused by gross negligence or willful misconduct. Her Prep Academy is not responsible for lost or stolen belongings.
2. Medical authorization
If my player is injured or becomes ill and I can't be reached, I authorize Her Prep Academy staff to give first aid and arrange emergency medical care, including transport to a hospital. I accept responsibility for any resulting medical costs. I've listed any medical conditions, allergies or medications in this form and will tell Her Prep Academy if anything changes.
3. Photo, video and media consent: basketball content only
Camps are filmed and photographed by our media team. I give Her Prep Academy permission to photograph and record my player during camp and to use her name, grad year, school, team, ranking, photos and video in basketball content only: drills, games, combine testing, on-court interviews about her game, highlight videos, game film and rankings. This content may be used for her highlights, shared with college coaches and scouts, and posted on Her Prep Academy's website, app and social media, including to promote Her Prep camps and the Her Prep app.
Her Prep Academy will never record or post anything that isn't basketball, and will never publish her home address, phone, email or birthday. No payment is owed for this use. To opt out of social media posts, email herprepacademy@herprepacademy.com. She'll still receive her own highlights and film.
4. Refund policy
Camp fees are non-refundable. With at least 7 days' notice, her spot can be transferred to another eligible player or credited toward a future Her Prep camp or event. If Her Prep cancels or reschedules a camp, you choose a full refund or a credit.
